The sizing is based on wearing it over a T shirt or a sweater. When you want to wear it over your jacket you might need a bigger size. Printed on the lining is Giels Blood Moon chopper, hence the name of the vest. You can read more on this chopper in 13 and a Half Magazine #4. Tight fit, so order one size up at least.
